Certificate Insurance for ECAC National Invitational Rowing Championships

has paid USRowing for 2008 annual membership fee. No further proof of insurance need s to
be provided to the QRA and ECAC.

Check if applicable (College or University Name)
has paid USRowing for 2008 annual membership fee, and has provided USRowing with a
Certificate of Insurance. Institution will provide the ECAC with a Certificate of Insurance. The
Certificate should include the following information:

• Eastern College Athletic Conference, Quinsigamond Rowing Association, their officers, directors, trustees, members and volunteers should be named as an additional insured.
• Coverage for watercraft liability and athletic participation should be indicated on the certificate.

We must receive these certificates by April 9, 2008. They should be mailed or faxed to:
